INTERGRATED TEAM CARE - (ITC)

ITC (previously Closing The Gap) is a national initiative aimed at improving the health, wellbeing, and life expectancy of Aboriginal and Torres Strait Islander peoples.

The program focuses on reducing health inequalities by addressing preventable illnesses, improving access to culturally safe healthcare, and supporting community-led solutions. Through partnerships between governments, health services, and Aboriginal and Torres Strait Islander communities, ITC works to create lasting change and ensure all Australians have the same opportunities for a long and healthy life.
