From the 1st of May management of the Close the Gap team will be taken over by Nepean Community & Neighbourhood Services. NCNS is thrilled to be part of the Close the Gap program, a program that supports Aboriginal & Torres Strait Islander people with chronic diseases.
The team is made up of Chronic Care Coordinators, Outreach Workers and Project Officers who work with GP’s to link clients to culturally safe healthcare services. This program helps Aboriginal people access healthcare early and continue to have regular health checks.
Aboriginal people are over represented in areas of chronic disease and this team provides important services that improve the quality of life for Aboriginal people with chronic disease.
NCNS is fortunate to have been able to transition the existing Closing the Gap team to our organisation. We are thrilled to have Veronica, Joanne, Steph, Sarah, Robyn and Shana continuing their important work, with clients who know and trust them with their care coordination.
This program builds on the significant Aboriginal support and services that NCNS provides. It fits with NCNS’s core values and dedication to improve outcomes for the Aboriginal Community in the Nepean area.
