Closing the Gap (CTG)  Integrated Team Care (ITC) are continuing to provide a service to the community with only 2 key changes with the way we work. We are currently not in the office, so we are working from home due to both Covid-19 restrictions and vulnerability of the community we service. This means that we are unfortunately not able to attend for home visits. What we will do in this place is arrange phone conferences.

In fact, if you are a current CTG client you should have already been contacted by your Outreach Worker on the number we have for you on file. We will discuss with you your upcoming appointments and even have a yarn as we understand the importance of staying connected.

In terms of transport we will be utilising both current running community transport services or taxis to get you to your medical appointments.
As for the administration and payment of medical bills, the process for this has not changed and the team are working hard behind the scenes to ensure all payments are going through on time.

Furthermore, We are also still open for all new referrals and allocating these accordingly each week. If you think you may need to be referred to our service please discuss with your local GP.
